Not an error. It is a vise job, PMD. Use the search box upper left of page for more info.

I think the only doubling here was putting two pennies in a vice - note the lettering is reversed. I am sure others will comment

Yes, vise job. Reversed lettering is a dead giveaway. Thus, suggest not submitting it for grading/attribution.

I was leaning towards it being fake but I figured I'd ask for a second opinion on this.

If it were double struck then the both sets of devices would be normal When looking at your coin the top ones are all mirrored on both sides of the coin. So it was transferred off another coin, not from a second strike of the die.
