Not an error, it has been damaged.
There is no way for that to happen during the striking of the coin.

Agree, not a mint error--post-strike damage. Coin didn't leave the striking chamber in that condition.

It is incuse. Punch on both sides which deform the coin. Nothing in the mint process could let same marks.

PMD as has been said. Nothing in the minting process would do this, but plenty of items in a garage full of tools can :)
