Is that what all these die gouges are from? I've seen this before. This one seems pretty extreme.

Yes, these are die scrapes, most likely from a feeder finger. They can get worse than this.
Error coin writer and researcher.

And they've been happening for a number of years. I have seen them from as early as 1938.
