Got this Nickel back in change recently and I'm wondering if this is just damage from circulation or an actual error?

All post-strike damage. Perhaps a mix of a stint in a dryer (the inner tub mechanism) and a boatload of circulation dings/hits/rubs.

Welcome here. Your coin is not damaged is very bed damage. By the surface of the coin I will say acid damage. The acid damage could be in the nature with low acids or in an intentionally workshop coin damage facilities.
