I think this is a Canadian coin. At least partly, I'm not sure about the back though or what the FD means before the date.

Anyone ever seen this or can they tell me what it is if not a Canadian cent/coin?

That is a coin from the UK, not an error. The obverse used the same portrait of our Queen as Canada used.
