Whatever caused it, it didn't look like that when it was struck.

I agree. It looks like an altered coin that says 198o-P instead of 1986-P. Looks like it might be a bit lighter than the 5 grams. If so it may have had a low acid bath from a possible soda drink for a short time. Thus etching the face of the coin. Just a face value coin now.
