It seems as if this coin spent too much time in circulation and got really beat up and dinged all over.

Not an error of any kind. This could not happen during the striking of the coin.
Just a badly worn and damaged nickel.

Heavy circulation on this one. Possibly vending or casino use for it's lifespan?

On my one trip to Vegas my wife hit a $100 jackpot on the Nickel-Slots that paid-off in coin, not an electronic jackpot. It seemed like most of the coins in her 'Big-Gulp' sized cup looked like this one. I had a nice score on the 'Wheel of Fortune' slot when I won 1,250 quarters ($312.50) and again, the coins were battered and bruised, but not to the same extreme as the nickels.

I think we were there just at the end of the solid coin payout era. As I understand it now, you put in a casino card and the cost of a play and your winnings (yeah, right) are all placed on that card.

Any of you 'Whales' out there know if I'm correct on this one?

yes, casino coin, the zillions of hits from other coins have flattened this one's letters, making them wider

Looks like the die was also a VLDS (Very Late Die State) to start with and the heavy usage of the coin just altered the surface event more. Spender.
