Since it lost over a third of its original weight, I'm going to guess sanding and/or acid etching.

Ok yes something isn't right for sure. With all that porosity visible, I wonder if it isn't a casting of one side of a nickel.

^This was my first impression. Either way it is most likely PMD. It also could be a split planchet, but the reverse surface doesn't look like a delamination.
