1984-D-Quarter,-No-Rim?
The coin is smooth on one side and has a rim on the other, I don't know if this struck off center. Also would be halpful to include worth if possible.

the design was probably ground off, so just a damaged coin that can be replicated at will, but without a pic tough to know for sure

Either a very minor MAD or just circulation wear.
