Thoughts About Coins Rotated Within A Slab

I recently saw a NGC graded coin that was significantly rotated (about 25 degrees clockwise) within its slab. A check of its certification number showed that it is a valid submission.

I'm wondering if it's common for certified coins to rotate within their slabs. If so, what causes the rotation?

Is it an older rattler slab? These often had the tiniest amounts of wiggle and over time the coin could rotate with those wiggles.

If its a newer slab where the coin is held in place with a rubberie-something, then it was very likely just bad QC by whomever stuck it in there.

I have a PCGS coin that has a similar problem. The holder is to blame for slab. Incorrect installation, slab parts are not fully fixed, there may be many reasons, but there are fewer and fewer problems in newer holders. Another problem may be that the holder is not matched in size.
