Is this a dual type error coin? Is it Cud error with Die rotation error (I measure 15 degree) or a Counterfeit? What are your opinions?

I believe that the quality control on those issues was rather poor with respect to the weight.

This is normal with this particular type of 200 cash coins.

Quality control barely exist and there are many varieties. These are struck in copper or brass or even overstruck over old Empire cash coins.

Those "patches" are known as " Cuds". That's where part of the die that struck the coin broke away, leaving a raised area with no design elements on the coin.
