I wouldn't know what was used. Something blunt and metal. I never bother picking apart damage, I just toss it back in the pile.

This looks like a slow, deliberate indentation by a punch to me - not a high-impact hit, as from a .22 or BB - which would leave more of a gouge on the obverse and a gradual indentation or bulge on the reverse.
