The hub broke off that device (Which was raised like a coin) and when the die was created, the incuse area where the breakaway happened, allowed that part of the device to be missing. This happens occasionally: A die chip will be raised as there was a void on the die. A broken hub will not show the normal part of the device, where the working hub chipped away.
