Digging through loose change, and found this (see picture). Weird. Is this a known error on a 2013 D Penny?

It is PMD not an error.

That is mechanical damage of some sort, First thought was from a coin rolling machine, but seeing how it is one continuous groove, I doubt that this is from coin rolling machines.

Yes, has been seen countless times--the damage caused by a mechanical coin wrapping device.
