I see a listing for a similar piece here:

They describe it as a "souvenir token", but that oblong hole makes me think that it was attached to a ribbon, or strap of some sort when it was issued. I'll keep digging to see what I can find.

I found a couple other listings across the interwebs, but no new information. I spent some time on the NSCDA website and took a couple virtual tours of their museums, but didn't see this piece on display. To further your research, you might reach out to the contact listed on their website. Thanks, Spence. I've dug and dug. At least I learned about the National Society of the Colonial Dames of America. They do like their medals!
There's one on ebay for $50. And I have at least a dozen of them.
