1994 P Dime With Errors? Need Help Understanding Problem With Coin.

This dime is odd. The obverse has what appears to have what looks to me like some type of doubling of his eye and brow. The reverse has a bump where e-plu is located opposite of what I think is the doubled eye and brow. The pictures do not have great detail. Also what is causing the coins damaged look? Any ideas?

Welcome to the forum! Looks to have been in a fire, the outer clad layer separates and bubbles up. Just a badly damaged coin.

Seeing the bubbling in front of Roosevelt and just opposite on the reverse just above EPU indicates high heat which separated the cladding from the copper core.

Definitely PMD, perhaps some road rash along with fire damage. Spend it or give it to a young numismatist to be.
