The cut in the neck and top of the head could have been caused by a Coin Wrapping Machine. This coin was at the end of the roll and the crimping jaw touched the surface of the coin as it rotated leaving these marks.

The rough fields are from the die being used beyond it's expected working life. They were just worn out.

Could be another aspect of this coin being struck from a tired die or circulation flattening or (most likely) some of both.
