It doesn't look like it's been tampered with or anything anybody have any ideas on this? This is an error or some kind of damage.

Yes, post-strike damage, likely of a blunt force nature.

You need to recalibrate and learn more about how coins are minted.

The coining chamber is round. Two dies strike the planchet and it's constrained by a collar (the 3rd side) that forms the edge.

Some coins have lettering applied to the edge in a subsequent step.

Nothing non-round will be "not tampered with".
