What happened to cause this damage..especially to the reverse. It looks like a chunk was taken out without collateral damage..I'm sure it's PMD. What do you think?

Just opposite that gouge, I can see a slight bulge (collateral damage)
The obverse gouges are damage as well, with bulging on the opposite side.
