It appears to my eyes that this is damage of some sort to the devices (not the big DDO)

I agree definitely not the 1969 S Cent with a dramatic doubled die, but could be the more subtle one. More likely though is Machine Doubling plus circulation flattening. For reference, here is the vv link:
