The reverse is almost certainly Die Deterioration Doubling, plus some of that ejection damage where the sides of the die cavities scrape the devices as the coin leaves the die.

I will punt on the obverse. Probably DDD as well, but let's hear what folks have to say.

I'm in with extreme Die Deterioration and the rims look a little wonky, like it may have had a short ride in a dryer.
