Hmm, From the picture shown, it does not appear to be an error, but instead post mint damage. One idea is if there are depressions on a coin like you have that don't look like they belong there, more than likely damage.
It is customary to post full, large and sharp pics of the obverse and reverse. Then, perhaps another image or two of the issue(s) in question. From the single partial photo, I'm thinking post-strike damage.
