Hello. I've only been collecting for a couple months now. The attached 2015-d Saratoga Quarter is what caught my eye and have been going through to learn more ever since. I'm still not sure what error this may be or what may have caused it if not. Is anyone able help with what this would be?

To CCF. It's PMD. The rim is pushed in from a hard blow.

What caused this? Maybe a hammer on the rim, heat left of sword and a lot of cars running over it when in the road.
