Looks like somebody tooled the reverse. This is not possible to happen at the mint correct?

Looks like the reverse has been machined away, likely one half of a Magician's coin.

Correct. The circular lines are a clear indication that the coin itself has been lathed.

Just like you can't clap with only one hand, you can't make a coin with only one side on it. A coin is made by pressing a blank piece of metal in between two dies. If one of the dies is simply missing, then there's nothing for the other die to squeeze against and no coin can be formed. The only way a genuine "one-sided coin" can be made in a mint is if someone deliberately makes a "blank die", and deliberately inserts that blank die into a coin press. It can't happen by accident.

Coins like this are often the first stage of production of a Magician's coin.

yes it is possible, but only by somebody outside the mint that has a lathe to carve out the design elements to make a Magicians coin.
