Given the posted pics, I'm leaning towards a Die Deterioration issue.

For some reason all the Patsy Takemoto quarter I have found the date look like that, so it seems like thats how they made them.

I don't believe a deteriorating die can thin out the numbers in this way. I suspect this is a striking issue, either a mild grease strike or just insufficient striking pressure.
