It's a severe case of Machine Doubling. It's a good example.

I think this is actually double struck. It has the appearance of flat-field doubling most notably on the right side of the letters in ELIZABETH II DEI.

You can see ghosts to the right of the letters in ELIZABETH. Double struck + MD?

PCGS say "Double Struck in Collar".
Pic is a bit on the dark side unfortunately.
