**Not asking about doubling**

What causes the letters to become like this? Incomplete Strike, Die Error, etc?

It looks like "push doubling", which is a form of Machine Doubling.

It almost looks like a Grease Filled Die, except it's way too precise and appears across multiple adjacent letters. I can't think of any other phenomenon than Machine Doubling, although there are certainly many rare and unusual types of minting problems cataloged on ErrorRef.com.

P.S. Excellent pictures, particularly the full coin pictures. It can be a bear to get proper images of proof and SMS coins.
