I've seen knifed coins that looked like this, they smooth over, over time. The "raised area" is actually pushed up metal that has folded over and smoothed. Even though this is an older coin that could do this, it's in a protected area, a little odd for this to happen but still think its PMD of some sort.

it looks like somebody scribes it with a sharp tool.
If you look closely, you can see a center line and the metal piled or moved on either side.
