What makes you think it is a wrong planchet?
Just a damaged quarter that has been ground.

This cannot happen during the minting process. All damage, regardless of how it happened.

It looks like it was sanded down to remove the rim and high points

Just a damaged Nickel from being sanded/grinded down. Worth 5 cents. Have you looked at the link I gave you on a separate thread about the minting process?
