Is This 1975 D A Major Die Break?

Kind of looks like a steel buffing wheel. But the defacer would know for sure what happened.

that could be a die adjustment strike but the chances are slim. I think it is mutilated because of the rim on the bottom (it's bent a little...)

There is no way that the coin could be a die adjustment strike. coincollector and others are right on the money. They considered the overall picture.

When looking at a coin like this, especially one with a bent up edge and rim, you should assume damage and work backwards to figure out if there is any possible way for a coin to come out of the Mint like this.

This coin doesn't get too far past square one:-) There is nothing here that resembles a Mint error.
