How could this happen and how did it get out of the mint? Really poor QC I say.

I originally read "struck through a steel washer" and the picture didn't make sense to me.

Still doesn't make sense! That's really really weird.

Could we see a pic of the sleb I have about a dozen od these, but would never have the nerve to have one graded. Cool, all the same.

I simply see this as a bored and unsupervised you S Mint employee with some time and ideas, doing what we are now seeing more of, what ever they wanted and I am sure they were removed from the Mint, easily.

I also am seeing more of these great strange, and planned examples of "errors' as some are called lately, more than ever.....I really do not think an employee could get these things out of the Mint, with out some supervisory knowledge or assistance....

There are probably a dozen such errors scattered over the last five decades of the 20th century. I have no reason to think they were intentionally created. Pieces of hardware (washers, nuts, broken screws) do wind up in our coinage presses from time to time. They either enter the production stream accidentally or fall out of the press itself. Of course, there is always the possiblity that a few of these were intentionally created, but there's no way to tell.
