We could X-ray the boxes and supply affidavits attesting that the image matches the box (like that would ever happen). I agree that proper modern QC should include a video record of both the coin(s) being placed in their OGP and the fulfillment process. It is so inexpensive these days to take and retain such videos and they should be used to shoot for at least a 6-sigma correct QC passed product. I am not advocating that such information be used against any individual employees, I know that mistakes are almost surely far more common than outright theft, but that such information be used to design procedures that catch and correct mistakes before they become the customer's problem.

Or weigh the box before it goes out and reject it if the weight doesn't match what should be in it. The only variable I can think of would be packing material but even that should be predictable given box size and dimensions of the product(s) inside.

I too thought about the QC's actions, I mean I bet that idea about the video's at the fulfillment center probably exists. I was just dumbfounded and am still dazed about the missing ASE........  I usually take pictures of the package that arrives mangled, THIS package was beat up and had extra clear package tape along the sides that were torn, but not that noticeable, until I opened the box and noticed the missing "big guy ".......Then I put all the facts together and called....... I hope Foxwoods Man and I have a smooth quick outcome.....

I think that with the current fulfillment contractor the packaging is better and more predictable than it was with PB. PB was less than expected, for instance, with the 2011 anniversary ASE set, they used the same box (apparently the only one they had that could handle the long OGP box) for one to five sets and filled with crumpled paper. The problem was that they did not use enough paper and the amount varied. My five sets weighed (according to UPS) 2.2, 2.3, 2.4, & two at 2.5 lbs. I opened the light one just in case things had rattled around but it was OK. There were lots of reports of coins out of the OGP and even coins out of capsules, which appeared to mostly happen during shipping.

Look at your two posted obverse images and key on the date. It looks like doubling on the 2, 0, and 6 AND some weird issue above the numbers like a hollowing out or something more apparent on the 2nd posted coin

I have re-read this topic and went to the Mint's on line site to check where my order is and when to expect them. I see the 6x6x6 package is at the mailbox. IF I posted the weight of that 1 package , I wonder IF people could guesstimate the amount I ordered. I ordered only the UNC ASE'S. The site reads that the weight of the single package is " Service FedEx SmartPost Weight 0.6 lbs / 0.27 kgs  Total pieces 1 " I am concerned that the weight and the actual quantity I ordered are off, somethings amiss.......... Any thoughts or guess's on how many Unc ASE'S are in the single package?

Yup, saw that (as I said). Not referring to the Photoshop component..(as I said)
Also..checked my order and two of the four have similar doubling of the date and the indents above the numbers. More prominent on the zero
