If I understand correctly, lamination errors occur when there is a defective planchet (impurity, inclusion, etc), which causes metal to flake off or peel off post-strike.

Seems to me this is likely what happened to this coin. I would guess that if this crater were present in the planchet before the strike, the result would have been an uneven distribution of metal near the defect, but a smoother flow of the metal around it than what is seen here?
AGain, this is an amateur's 2-cents worth, and it would be interesting to hear one of our error experts chime in.
