a closer inspection of the reverse, it does appear as the fifth hole is a stain...

My guess is it is a counterfeit and the holes were put in it to keep it from getting passed off to someone else.

My first thought was a kids whizzer toy... no not the cleaned coin type but the string toy made for amusing kids. Drill a couple of holes and add a string to twirl around your head to make a buzzing/whirling noise.

But then I realized it had 4 holes and positioning made me think charm bracelets...

S mint mark? no clue...

I did an overlay of all four 1909-S V.D.B. Dies. Mint Mark #1, #2 and #3 show "large" areas of the Mint Mark would be visible on the holed coin. Die #4 was harder to see, because almost all of the mint mark would have been removed by drilling a hole in this area. However, a SMALL area would still show the the serif on the bottom left of the "S" Mint Mark.

There were four different mintmarks on the 1909 S VDB cent. The first three are definitely ruled out. If and only if it were it would have been the mintmark from die number four. Unfortunately there will never be a way to confirm it.

Just a Wheat cent not. It is a cull coin. The value went out the hole.

I'm thinking jewelry attempt. People did and do a lot of that type of thing. It was probably done for some grandson/daughter and grandpa just used some of his old pennies he had in a jar in the garage... The kid was happy... we however — are all crying! : (
