Definitely not an error. It's Post Strike Damage ( PSD). This has happened after it left the U.S. Mint (after the strike). Someone either cut or punched out the metal.

Done by a Coin cutter for jewellery purposes .
Very common at flea markets or artisan fairs.

Holes have been punched out with punches specifically made for the purpose. Notice how on the obverse the edges of the hole curve downward into the hole, but on the reverse they are sharp and square.

Sometimes they are cut out with a coping saw/wire barbed file. Clad or silver? (I'm guessing silver as it was probably intended at a piece of jewelry?)
