I believe it's from Philadelphia. The D on the reverse is maybe part of the designer initials?

Unfortunately, its not some rare error, its just the designer Cyrus E. Dallin's initials

So you all don't think the obverse was minted in Philadelphia, then the coins were shipped out to Denver to complete the process....  |

It's also a bit odd that the coin bears the initials of two artists and two sculptor-engravers.

There are 4 sets of initials on the quarter. RS and EC on
the obverse and the initials D and PH on the reverse.

Come to find out...Cyrus E. Dallin is dead, he died in 1944. But apparently he designed the Pilgrim Tercentenary half dollar and I guess they used the likeness of the ship he designed for the half dollar on this quarter, which is why his initials are on it...  |
