Sorry all.
I should have been clearer and given details like enworb.
The Queens Visit one was the one I spotted today.

'If my memory server me correctly', Vladimir Gottwald designed the 2001 Queen's Visit Australia only obverse. Was submitted to the Queen for British Commonwealth general issue. Was rejected by her for the I.R.B. design, but was accepted later for the the 2001 issue only.

I have taken this question without notice without confirming my reference.

The Royal Visit coin is 2000, and that effigy was only used for that coin, thank goodness. Probably the 2nd worst image of QEII I've seen on a coin.

You have to go out on a limb once in a while to get the blood flowing, but having said that I doubt that you are wrong

Ugly refers to it as "the cleaning lady portrait"

Four, though there seem to be a few sub-types if you want to get technical.
