To me it looks like the coin was struck over the top of the metal flaw.

This coin was struck on a laminated planchet, it looks like the laminated piece detached itself after striking...

I've never seen anything like that! I would not have expected so much detail to be left behind after a piece came off the coin.

Also, I think it was very considerate of the error not to mess the date up. LOL
