Is this an RPM, or something else?

It does not appear to be re-punched, so I figure maybe deterioration?
Or is this normal?

That's how we learn.
I've been seeing that mm strike style (top left corner is wider than bottom left) a few times and been wondering if it's normal.

Another question answered, thank you!
