It is dug into the coin and NOT raised, Correct?

If it is NOT raised then its one of three things, usually
The gumball or toy machine
An auto quarter wrapper machine
The coin got stuck in a coin Counter

The guys above pretty much nailed the issue if the coin is damaged.
