With that dark center and what looks like melt through, I wonder if someone stuck it in a electrical outlet or did something electrical related.

if this was caused by an electrical short, you would see arc marcs, or if part of the circuit and solidly attached, most probably no marks.
So, the electric theory can be ruled out.
