This coin has the reverse on both sides. Thickness 255.09 mm. Weight 4.7g.

It is called a Magicians coin. You can buy them at magic shops. On the first image you posted, you can see where the reverse from another coin was added to the altered coin. Note your last image. The area between the rim and the USA, you can see the area in question. Good for a coin toss as most people usually pick heads.

Is it just me, or does the first picture look like the rim's been cut all the way around?

It's funny the U.S. Mint agrees this practice is o.k., I think in Canada defacing money is a penalty of some kind.
