1994-D Quarter Error Or Altered

1994D quarter

Is this an error or altered?

Three (3) solid raised five (5) point stars on obverse

Denoted in red arrows:
On neck between 9 and 4

Denoted in yellow arrows:
Left of neck above 1

Denoted in blue circle:
Above mint mark

All are noticeable by the unaided eye. All appear raised iv viewed from the side. I found the one in the neck first, the one on the left second, and couldn't tell what the one above the mint mark was.

All are obvious with a magnifier. About 250% zoom on a computer.
