Its not an error. First thing came to mind was it was attacked by an ice pick just PMD

I don't know why someone would attack it with an ice pick, but that does appear to be what happened.

I don't know about that. This coin is over twenty years old. Plenty of time for a breach in the copper coat, leading to zinc rot and more holes from the inside.
