To me it looks the Die has started to deteriorate!

If the die was deteriorating the "1" would spread, not get smaller. You are left with partially grease filled or, as stated, over polishing. I'd go die polishing as the number right next to it looks a little polished off also,

I think your right Mark. Looks like a little MD on the "0" as well.

That could also be from over polishing - think of a beveled edge on the die instead of a sharp one.

Partial Grease Fill on those devices that prevented the full strike of those digits.

Possible wipe with a cloth that moved grease into the affected devices.
