Is this an error - looks like Jefferson gained 50 pounds. And there's doubling on both the obverse and reverse.

Your Nickel shows severe Die Deterioration Doubling, caused by a very worn tired out die. The die was in a Very Late Die State (VLDS) when it struck your coin. This is a die wear issue. You can see the mushy details, and the heavy die flow lines going outwards. This die is at the end of its life, and should be retired now. There's no premium over face value for this, but you can keep it as a learning piece. http://www.error-ref.com/die-deteri...ion-doubling

Yeah he has a double chin now - looks cool though. On the reverse the doubling is just MD.

Yes, it's caused by a LDS/VLDS. Not a doubled die.

So what it is is both Die Deterioration and Machine Doubling. A double whammy, but not worth a premium. More of knowing what it is thing.
