Not an error. It's damaged from Post Strike Damage ( PSD). Something hit the other number perfectly, causing the metal to get completely pushed over and flattened.
This once was probably clipped with a coin wrapper that damaged the '5'. Possible the last digit was altered with a vending machine. So it may have been beat up twice.
