Please advise if this RPM is of anything of rarity and the presidium if applicable.

Looks MD at the bottom of the mint mark and a hit at the top of it to me.

I tend to agree with @john1 on this one--I don't think that it can be 1964-D RPM-004 since the mintmark is in a different location.

As mentioned MD. Note the other devices on that area shows the same event that happened to the coin after the strike. Either by the machine or by circulation damage. Not a RPM.
