Good question because at first glance it surely does look like PSD.

I'm not sure if malfunctioning equipment might have had something to do with the final result and not sure it could be proven if it was mint equipment.

It didn't get compressed from opposite edges or they would have a flat spot.

The tapered flatness seems to originate at K9 (LIBERTY). I wonder if this could be the result of a malfunctioning machine trying to produce an elongated coin. Getting jammed up and debris in the chamber?

That's very unlikely to have happened during the striking of the coin
but it could have gotten jammed in something else. PMD
