I'm going to have to disagree with you respectfully I'm afraid, though I do see where you're coming from.

Errers and Varieties gave this to me, so I think it's pretty assured that it is legitimate, from my point of view. The entire surface is raised, there are no indents around it, and the reverse is clean.

Looks like a progressive die chip. It has been crumbling now for sometime and will probably continued until the die is retired.
